Confidentiality is absolutely crucial to the process of clinical trials. We will do everything we can to insure that your information remains completely private. 1. When you first contact us, one of the first things we will do is gather "contact information" from you: When and where we can attempt to contact you, as well as where you do not want us to attempt any contact. 2. Your charts here are secure. Our cabinets and facilities are always locked and secure whenever the office is vacant. No person except staff has any access to where charts are stored. 3. You records will be identified with initials and a case number assigned to you according to the protocol. However, the FDA, the Biomedical Research Company and the Institutional Review Board (IRB) do have access to your records which identify you. This is done in order to insure your safety and well-being. For example, if a year down the road a new side effect emerges, you might be contacted to inquire about whether you are experiencing any such problems. 4. It may well be that your records with us are more confidential than your records with your private physician or hospital. In most cases with your PCP or hospital, you need to sign a release for insurance companies to have access to your records - no matter what they contain - or the insurance company may not pay out any benefits. That does not occur here. You will not be asked to sign any such release. 5. Your name and address will never, under any circumstances, be shared with any outside institution or mailing list. 6. When you use our website, we do not insert any "cookies" or other electrical information on your computer. We do not keep a record of who visits our website. Self-testing materials you might access through various links from our site will not be recorded any where on our website. However you should review the privacy policies of each website where you might undertake any self-assessment. 7. Only a court of law or an overseeing regulatory authority, such as various state-mandated regulatory boards, are allowed any access to your records. This is a very rare occurrence. In the event that such an event were to happen, you would be contacted concerning all particulars. 8. If, in the future, you want specific information released or letters written to treating physicians (for example) you will be required to sign a specific release for each record released. |
